Instructions
1. There are two versions of
the Tournament Sanctioning Form available.

Use
the Word Version with macros to complete the form on
your computer.

Use the
PDF Version if you want to print off the
form.
2. Fill in all required
fields.
3. Sign the “Tournament
Chairperson” section.
4. Either contact a regional
representative to get them to sign or email it to lesley.squibb@ontariobroomball.ca
to obtain a
signature.
5. A FBAO Executive member
will then authorize the tournament sanctioning information and, once complete, your tournament will be officially
sanctioned.
Note : The form
must be completed and submitted to the FBAO Secretary at the address below 15 business days prior to the event
taking place. Incomplete information could result in delays in determining if the event will be
sanctioned.
